High school diploma or equivalent, prior experience working with the frail and elderly population, and 2 years' experience in Medicaid eligibility, financial determination, or benefits coordination
RN - Admissions
Graduate of an accredited school of nursing; 1-2 years of experience in hospice and/or palliative care, medical/surgical, public health, oncology, acute care or home health; current Colorado RN licensure
Personal Care Associate - PACE
High school diploma, 1 year of experience, valid Colorado driver’s license, and proof of auto insurance
Medical Assistant - PACE
1 year of experience, current Certified Medical Assistant Certification, valid Colorado driver's license
LPN - Inpatient Care Center - Nights
Graduate of an accredited school of practical nursing. One year of experience in hospice, palliative care and/or home health, medical/surgical, public health, oncology, acute care or home health
Palliative Care Navigator
Minimum 2-5 years of experience in a clinical setting and a Bachelor's degree preferred
LPN PACE
One year of experience in hospice, palliative care and/or home health, medical/surgical, public health, oncology, acute care or home health
Clinical Manager - Inpatient Care Center
RN license required, 3 years of supervisor experience in healthcare, Bachelor's degree in health care, business or related field
