Job Description
The Denver Hospice has an immediate opening for a Palliative Care Navigator. The successful candidate will manage scheduling for NP providers, assist with patient care coordination, and oversee administrative tasks for the Palliative Team.
Requirements
- Bachelor's degree preferred; Degree in Behavioral Science, Nursing, Public Health or similar field desired
- Two-five (2-5) years working in a clinical setting
- Ability to lift/carry a minimum of 30 lbs
