Job Description
This transitional care nurse position requires a Registered Nurse to provide specialized care to individuals with disabilities and/or acute/chronic illnesses under the supervision of the Nurse Team Lead. The role involves performing hands-on nursing care, coordinating education, advocating for patient-centered goals, and ensuring safety standards are met. Mission and customer service are key.
Requirements
- Licensed as a Registered Nurse in the State of Montana
- Minimum of a Bachelor’s degree in nursing preferred
- Willingness to obtain certification in rehabilitation nursing within the first year of employment
- CPR certification
- Three years’ experience related to the duties specified
- Ability to function independently in performing assigned duties
