Job Description
Supportive Housing Case Manager will work with a caseload of 10-20 single and family households in supportive housing, using a strengths-based approach to perform assessments, develop individualized service plans, and provide support to clients experiencing crises.
Requirements
- Bachelor's degree in human services or related field
- 1+ years of case management or direct services experience
- Strong written and oral communication skills
- Ability to prioritize competing priorities and make sound judgements
- Valid Driver's License and reliable transportation
