RN Case Manager - PRN

Sanctuary Hospice is seeking a PRN Hospice RN Case Manager (RNCM) to provide exceptional end-of-life care to patients and families in Summit, Stark, Wayne, Southeast Medina, and Western Portage Counties. As a valued member of our interdisciplinary care team, you’ll help ensure every patient receives The Best Care Possible.

Requirements

• Current unencumbered license as a Registered Nurse
• Minimum one (1) year experience as an RN in a medical surgical/acute care setting
• Computer proficiency in typing and various programs, including background in EMR
• CPR certification
• Current driver's license, vehicle insurance, and access to dependable transportation
