Job Description
We're building an affordable health plan designed around Sana Care, an integrated care model connecting members with unlimited primary care and expert care navigation. The Care Navigator will work closely with patients, providers, and other members of the healthcare team to ensure that patients receive the highest quality of care.
Requirements
- 2+ years work experience
- 1+ years working in healthcare
- Prior Care Navigation experience
- Excellent communication skills
- Ability to build relationships and interact with people at all levels
