Job Description
Porter Cares is expanding into San Antonio, TX and is seeking Nurse Practitioners to provide in-home health risk assessments. The role involves visiting members, conducting comprehensive physical exams, point-of-care testing, and formulating medical condition lists to support care management and treatment.
Requirements
- Valid unencumbered Texas NP license
- 3 years of patient care experience required
