Case Manager

Responsible for conducting case management assessments, developing individual service plans, and assisting with program placement. Coordinating delivery of support services for clients and monitoring progress towards achieving service plan goals.

Requirements

• Bachelor's degree in Social Work, Psychology, Counseling, or Social Service field
• 4 years experience in case management
• Ability to work independently and in a team environment
• Fluency in Spanish a plus
