Pathways Home Health, Hospice and Private Duty

Pathways Home Health, Hospice and Private Duty is seeking a Physical Therapist to provide care at home or in settings such as assisted living, a nursing home, or the hospital. The Physical Therapist will evaluate and treat patients using medically prescribed physical therapy programs, with a focus on restoring function and preventing or minimizing disability.

Requirements

• Graduate from an accredited physical therapy program
• California license as a Physical Therapist
• Minimum 2 years recent experience as a Physical Therapist
• Current CPR Certificate
• Current California driver’s license and automobile insurance
