Health Care Guide I

This Health Care Guide position provides support to members referred to the Care Coordination department for Case Management services. The role involves assisting members, families, and providers in coordinating benefits, researching resources, and communicating with various agencies. The Health Care Guide works collaboratively with the care coordination team to ensure effective benefit administration.

Requirements

• High school diploma or equivalent
• Two (2) years of experience working in a health care setting
• Experience in case management or care coordination preferred
• Valid California driver’s license and current automobile insurance
• Proficiency in computer software for word processing, spreadsheets, and email
• Basic knowledge of medical terminology and coding
• Ability to use judgment and exercise discretion
