The role involves assisting members, families, providers, and community agencies with benefits coordination, resource identification, and communication. Responsibilities include serving as a point of contact, initiating referrals, and conducting assessments to identify care coordination needs.

Requirements

• Act as the point of contact for members and other parties.
• Initiate and process referrals in accordance with department policies.
• Collaborate with members, families, providers, and community agencies.
• Research and provide resources for care coordination.
• Complete documentation accurately.
• Provide excellent customer service.
• Utilize computer software.
