Navigator - Integrated Seniors Hub

Partners Community Health (PCH) is seeking a Navigator to support the development and implementation of a plan for access and system navigation to seniors services in alignment with community partners and applicable legislative requirements.

Requirements

• Registered Nurse or a member of a regulated health professional college
• Minimum of 2 years of recent experience in community health or a related field
• Knowledge of the health care delivery system and community resources
• Excellent interpersonal, communication, assessment, critical thinking, problem-solving, and decision-making skills
