Job Description
Pair Team is a tech-enabled medical group reimagining Medicaid and Medicare to provide whole-person care. They partner with Federally Qualified, Non-Profit Health Centers in California to provide integrated care, specifically enhanced care management for individuals with serious mental illness, substance use disorder, or high medical needs. They are seeking a Bilingual Family Nurse Practitioner to engage and support patients in a virtual setting.
Requirements
- 2+ years of clinical experience in serving patients with complex social and healthcare needs
- 1+ year of primary care experience
- Experience or desire to work closely with a multidisciplinary team
- Board certification or eligibility in Family Medicine
- Active DEA license preferred
- Bilingual, Spanish and English speaking
- NP license in CA
- Startup experience is a plus!
