Job Description
Pair Team is a tech-enabled medical group reimagining Medicaid and Medicare care. They aim to build a care model focused on compassionate, high-impact care and empowering clinicians. They provide a full-spectrum of services for underserved populations, including mental health, substance use disorder, and homelessness, and are looking for a highly motivated Bilingual Family Nurse Practitioner to contribute to their new enhanced care management program.
Requirements
- 2+ years of clinical experience serving patients with complex social and healthcare needs
- 1+ year of primary care experience
- Experience with a multidisciplinary team
- Bilingual in Spanish and English
- Board certification or eligibility in Family Medicine
- Experience working with Medicaid/FQHC clinics
