Home Health Social Worker

The Home Health Social Worker is responsible for providing psychosocial services to terminally ill patients and their families. This includes conducting initial assessments, referring to community resources, and providing counseling services. The position involves working with patients, families, and caregivers to address their needs and provide support.

Requirements

• Master's degree in social work from a school of social work accredited by the Council on Social Work Education
• Valid driver's license and reliable transportation
• Active Ohio Social Work license
• Minimum of 1 year experience in a health care field with experience working with patients and families coping with terminal illness and dying
