Home Health & Hospice Social Worker

The Home Health & Hospice Social Worker is responsible for providing psychosocial services to terminally ill patients and their families, conducting initial comprehensive assessments, referring to community resources, and providing counseling services.

Requirements

• Master's degree in social work from a school of social work accredited by the Council on Social Work Education
• Valid driver's license and reliable transportation
• Active Ohio Social Work license
• Minimum of 1 year experience in a health care field with experience working with patients and families coping with terminal illness and dying
