Home Health & Hospice Social Worker

The Home Health & Hospice Social Worker is responsible for providing psychosocial services to terminally ill patients and their families, conducting initial assessments, and facilitating referrals to community resources. The role requires effective communication, comprehension, documentation, interpersonal, and computer skills, as well as the ability to work in a field setting and make sound judgments.

Requirements

• Master's degree in social work from a school of social work accredited by the Council on Social Work Education
• Valid driver's license and reliable transportation that is insured in accordance with Ohio requirements
• Active Ohio Social Work license
• Minimum of 1 year experience in a health care field with experience working with patients and families coping with terminal illness and dying
• Hospice or home care experience preferred but not required
