Case Manager

New Oakland Family Centers provides comprehensive mental health care in outpatient, partial hospital, and crisis settings across Michigan. The Case Manager provides person-centered support, facilitating assessments, care planning, and advocating for clients in accessing services. The role focuses on providing continuity of care, crisis intervention, and promoting well-being within a collaborative environment. The position requires strong communication, multitasking, and a commitment to quality service.

Requirements

• Bachelor’s degree in social work, psychology, counseling, or a related human services field
• 1–2 years of experience in behavioral health and case management
• Valid driver’s license with approved driving record and auto insurance
• Reliable personal vehicle for travel
• Reliable access to electronic medical record systems
