Job Description
The Wellness Visit Coordinator will be responsible for completing pre-visit planning, identifying patient needs, and providing education on conditions. They will also support Value-Based Care initiatives, Lifeline enrollment, and Team-Based care. This is a full-time position.
Requirements
- Current Unencumbered Colorado RN license or Compact license
- Current Unencumbered Colorado LPN license or Compact license
- Registered or Certified Medical Assistant
- Current BLS certification
- One (1) year nursing experience in primary care, community nursing, critical care, nursing home, or home health, preferred
