Care Coordinator- SA Community Care

Join Medibank's Amplar Health team as a Care Coordinator in SA Community Care. Coordinate care and services to support patients in their own home, connecting patients, families, and providers. This role involves conducting initial phone assessments, care planning, and goal setting, as well as coordinating in-home services and managing escalations.

Requirements

• Registered Nurse with current unrestricted AHPRA registration
• Experience working in an acute setting (community/hospital)
• Strong understanding of care coordination, case management, or complex care
• Digital proficiency and ability to work virtually
• Excellent communication and relationship-building skills
• Confident working independently while collaborating within a multidisciplinary team
• Highly organised with strong clinical judgment and problem-solving skill
