Case Manager- Countryside

The Case Manager is responsible for the coordination and provision of service delivery to meet the needs of individuals served by the agency, including advocating for participants and ensuring daily needs are met.

Requirements

• Coordinate interdisciplinary team process
• Serve as advocate for participants
• Maintain individual case files
• Input data and maintain computerized case files
• Conduct pre-staffing meetings
• Ensure participants have supports and resources
• Conduct monthly reviews of participant status
• Coordinate data collection for statistical reporting
• Ensure daily needs of participants are met
• Participate in trainings and meetings
• Create and deliver individual or group skills training
• Serve as advocate for people with disabilities
• Maintain QIDP status
• Maintains American Heart Association certification in First Aid and CPR
• Successfully completes Little City Foundation required training
• Aid in the oversight of whereabouts of participants
• Notify supervisor or safety representatives of any violation of safety rules, hazards or barriers to participant wellness
• Perform other reasonable duties as assigned
