Job Description
Homeward is a healthcare delivery platform addressing healthcare access issues in rural America by aligning incentives for providers and ensuring quality patient outcomes. They focus on building partnerships with providers and communities to improve clinical outcomes and reduce health disparities. This role involves consulting, lead generation, implementation support, and relationship management with physicians and clinics in Michigan.
Requirements
- 3–7 years’ experience in provider relations, clinical outreach or value-based care delivery
- 2+ years of experience in lead generation, business development, or liaison/representative roles with physician practices and clinics
- Functional understanding of provider practice operations, revenue cycle, and value-based care
- Excellent presence and ability to build/maintain relationships
- Strong independence, autonomy, and initiative
- Bachelor’s degree required; MPH, MHA, RN, or equivalent experience preferred
- Travel regularly to rural locations across Michigan
