Job Description
The Social Worker will provide psychosocial support to patients receiving or recently admitted to hospital-based care. This role involves assessment, discharge planning, intervention, referrals, financial assistance, and trauma support. The position requires a graduate of a recognized program (LMSW, LSW, or LCSW preferred) and a commitment to providing trauma-informed care within a multi-specialty setting.
Requirements
- Minimum two years of post-graduate experience as a social worker
- 6 month hospital based orientation process
- LMSW, LSW, or LCSW licensure required (for New York State)
