Job Description
Guthrie Community Hospital is seeking a per diem Social Worker to provide psychosocial support to patients receiving or recently discharged from hospital-based care. The role involves patient assessment, intervention, discharge planning, and collaboration with the healthcare team. It requires experience with a similar healthcare setting and adherence to regulations. The position supports patients in transitioning to skilled nursing, rehabilitation, or other facilities.
Requirements
- Minimum two years post graduate experience as a social worker in a similar type of health care setting.
- Successful completing a 6 month hospital, based orientation process.
- LMSW, LSW, or LCSW is required for New York state staff.
- Experience with trauma patients and/or other crisis situations is preferred.
