The Peer Recovery Coach provides individual and group-based mentorship and support to individuals recovering from mental health and substance use disorders at the Henderson Detention Center and our Henderson office location.
Requirements
- Manage incoming requests and inbound referrals, including calls, emails, pre-enrollment submissions, and walk-ins.
- Completes various assessments, surveys, interviews, and other data collection methods to track outcomes of members for grantors, management, and community partners.
- Educate participants about peer recovery support and programs available at the Foundation, other treatment and recovery options, overdose prevention tools and strategies, and local resources.
- Data entry and recording in case management system
- Coordinate ancillary services for members such as: transportation assistance, vocational training, stable and secure living environments, and other recovery support services, as identified by members
- Facilitate whole health and recovery support groups, maintaining fidelity of the curriculum and program
- Support members with identifying whole health and recovery goals and developing plans to achieve those goals
- Support members with the planning of their recovery program, and provide motivation to assist members with completion of the program
- Provide individualized support, empowerment, and encouragement to members through coaching and everyday interactions
- Offer recovery management strategies and tools; like journaling, breathing and mindfulness techniques, among others
- Offer overdose prevention strategies and tools; like Narcan and fentanyl test strips
- Listen to members' barriers; offering assistance to remove barriers through linkages to resources and other programs at Foundation for Recovery - or just to listen and provide understanding and empathy
- Share and model personal resilience and recovery experiences, when appropriate, acting as a role model for members
- Provide regular communication to members through phone, text, and in-person follow-ups and check-ins
- Maintain clear and concise documentation using the organization's case management software
- Complete all necessary data collection, reporting, and documentation related to the program requirements
- Prepare program, grant, and caseload reports as needed
- Understand and adhere to all state and federal confidentiality laws and regulations
