Care Coordinator, East Street Living

Care Coordinator provides care coordination services to young adults living in the program, advocating for residents and their families in navigating human service and similar departments, agencies, and services. Collaborates on cases with a multitude of providers to ensure appropriate delivery of services to resident.

Requirements

• Bachelor’s degree in a human services field
• Experience in child welfare, youth programming, youth development, and/or related fields
• Excellent communication skills, both verbal and written
• Knowledge of and sensitivity to cultural, ethnic, racial and socioeconomic issues
• Possession of a valid driver’s license, reliable transportation and proof of automobile insurance and registration
• Bilingual/ASL skills
• Multilingual skills
