Licensed Practical Nurse (LPN)

The Licensed Practical Nurse (LPN) role focuses on enhancing patient outcomes through effective chronic care management and departmental quality improvements. This position will optimize Medicare Wellness visit compliance, address care gaps, and facilitate smooth transition of care, including hospital follow-up. LPN will also be responsible for medication adherence, referrals, and chronic care management services.

Requirements

• Schedule Medicare Wellness Visits in compliance with CMS guidelines.
• Scan upcoming appointments and obtain medical records.
• Manage time effectively and work independently.
• Communicate effectively with patients and team members.
• Maintain accurate documentation in Epic EHR.
• Administer injections/immunizations and provide basic patient care.
• Navigate the EHR system.
• Active Virginia LPN License and AHA BLS/CPR certification.
