RN - Nurse Care Coordinator

We are seeking a Nurse Care Coordinator to join our Regional Case Management Team. This individual will work closely with the behavioral health provider, primary care providers, and community agencies to provide professional assessment, coordination/planning of multiple health care services, and ongoing evaluation of care management services.

Requirements

• Graduate of accredited nursing program
• Valid license to practice as an RN in the State of Ohio
• Experience providing nursing services to persons with severe and persistent mental illness
• A car and valid Ohio Driver's License
• Knowledge of nursing rules and regulations in the State of Ohio
• Excellent organizational, attention to detail and communication skills
• Ability to work in a team environment
• Commitment to confidentiality, following rules of HIPAA
• Proficiency in Microsoft Office
• Skill in managing time and meeting deadlines
• Crisis stabilization techniques
• Ability to work non-judgmentally with individuals
