Job Description
The Path 2 Case Manager will provide in-home case management services for at-risk families in the Contra Costa County Children and Family Services (CFS) Bureau's Differential Response (DR) program. The primary goal is to help families address issues and avoid further need for interaction with CFS.
Requirements
- Equivalent combination of education and work experience is considered (minimum should have Bachelor's from an accredited college or university with a major in social welfare, sociology, psychology or a closely related field)
- Three (3) additional years of related full-time experience, or its equivalent, which includes at least one (1) year as a home visiting case manager in a public or private human services agency
- Bi-lingual Spanish highly preferred
- Knowledge and skills of principles, models, and techniques of intensive case management as relevant to children, youth and families affected by violence
- Knowledge of Contra Costa County community resources is highly desirable
- Valid driver’s license, proof of insurance, and access to reliable incurred vehicle
