Job Description
The Path 2 Case Manager will provide in-home case management services for families experiencing multiple stressors impacting their ability to parent effectively, helping them secure needed services, improve family functioning, and avoid further need for interaction with Children and Family Services.
Requirements
- Conduct in-home family assessments and follow-up assessments every three months
- Engage families in the development of action plans and goals
- Provide families with referrals to community-based resources and coordinate follow-up and documentation
- Attend mandatory Transition Meetings and provide 24-hour on-call support for families in case of crisis or emergency
- Maintain complete and accurate client service documentation in databases required by Contra Costa County and Catholic Charities of the East Bay
