Job Description
CareHarmony is seeking a Remote LPN (LVN) with at least 3 years of patient-facing experience. The role focuses on value-based care management initiatives for patients, including resource identification, medication management, and patient education. - Active Multi-State/Compact License (LPN) (NLC) (LVN)
- Technical aptitude – Microsoft Office Suite
- Excellent written and verbal communication skills
