Job Description
CareHarmony is seeking an experienced Licensed Practical Nurse (LPN) or Nurse (LPN) (LVN) to provide value-based care management for patients in a fast-paced, remote environment. The role focuses on identifying and coordinating resources, providing education, and assisting with medication management. The company offers a fully remote position with a rotational on-call schedule.
