Job Description
This full-time position involves providing palliative care for hospitalized patients and their families facing serious medical problems. The pharmacist will participate in clinical evaluations, medication management, and collaboration with other healthcare professionals to ensure appropriate and safe patient care. Training and mentorship are key responsibilities.
Requirements
- Active New York State pharmacy license in good standing
- Two years of post-graduate residency/fellowship training
- Earned certification in a clinically relevant area (e.g., oncology, hospice, geriatrics)
- Registered Nurse License in Good Standing
- Board Certification in relevant field
