Job Description
This role is for a Patient Navigator at AxessPointe Community Health Center, Inc. (API). The navigator provides direct customer service to clinic patients and visitors, coordinating appointments and managing patient files. The role requires a high level of customer service and a focus on both clinical and administrative tasks. API focuses on Team-based patient care with stated objectives and outcomes.
Requirements
- Minimum of a High School diploma or equivalent
- 1-3 years’ experience working with underserved/vulnerable populations
- Ability to operate computer and other office equipment
- Valid driver’s license and reliable transportation
- Flexibility to work evening and Saturday hours as needed
