Job Description
Advocates is seeking a motivated and compassionate Senior Case Manager to provide therapeutic and recreational services to individuals with autism within a developmental skills training program. This role focuses on fostering independence, skill development, and fostering growth through collaborative teamwork. Advocates is dedicated to creating a supportive and inclusive work environment.
Requirements
- Provide case management and skills training support to individuals.
- Develop, implement and monitor collaborative therapeutic service plans.
- Assess individuals’ behaviors and complete daily data and progress notes.
- Foster community integration and provide support to individuals to achieve their goals.
- Valid driver's license and ability to transport clients.
