Targeted Case Manager

Advanced Behavioral Health, Inc. is looking for a Targeted Case Manager to provide support and guidance to young individuals facing various challenges such as behavioral problems or suicidal ideation. The role will involve assessing needs, developing comprehensive care plans, and connecting clients with resources and services to promote their overall well-being and success.

Requirements

• Bachelor’s degree in Psychology, Social Work, Counseling, or related discipline
• Previous experience working with youth populations preferred
• Knowledge of clinic territory, community resources, and social service agencies
• Valid Driver’s License and reliable transportation
• Ability to manage, delegate and manage multiple tasks with completing deadlines and priorities
• Excellent active listening, communication, and documentation skills
• Strong interpersonal and customer service skills are essential
• Ability to work independently and collaboratively in a fast-paced environment
• Strong organizational and prioritizing skills
• Bilingual – fluent in English and Spanish preferred
• Empathy, patience, and cultural sensitivity when working with diverse populations and families in crisis
• Flexibility to adapt to the evolving needs of clients and program requirements
