Social Worker

VITAS Healthcare is seeking a full-time Social Worker to provide emotional and logistical support to patients and their loved ones during end-of-life care. The role involves ensuring patients reach their wishes regarding finances, funeral arrangements, and navigating the grieving process. The social worker will work in a variety of settings, providing care in patient's homes or in a licensed home setting.

Requirements

• Ability to travel per job requirements.
• Thorough knowledge of community resources or the ability to obtain them.
• Knowledge of psychosocial issues prevalent with terminally ill patients and their families.
• Capacity to relate and work well with an interdisciplinary team.
• Excellent listening and communication skills.
• Commitment to patient welfare and a Do-Good Attitude.
• Licensed Master Social Worker (LMSW) license (required for some positions)
