Case Manager - Primary Care/Pediatric Office

Community Health Center of the North Country seeks a compassionate and resourceful Case Manager to join their Federally Qualified Health Center’s integrated care team.

Requirements

• An associate degree in a health or human services field is preferred; some related experience required.
• Knowledge of available community resources and their organization, purpose, and use is preferred.
• Valid New York State Driver’s License with an acceptable motor vehicle abstract.
• Reliable personal vehicle for business use
• Ability to lift and carry up to 25 lbs.
• Excellent hand-eye coordination.
• Ability to sit for long periods of time.
• Ability to tolerate extensive exposure to a computer monitor/screen.
• Ability to tolerate extensive use of computer keyboards
