Requirements

• Responsible for rounding and managing the clinical care of a population at an assigned SNF.
• Responsible for delivering high quality, patient-centered, comprehensive primary care services to residents.
• Timely documentation of patient care/encounters.
• Availability for phone/secure messaging consultations with facility nursing staff.
• Reliable transportation.
• On-site presence during scheduled shifts (mandatory).
