Job Description
Texas Children’s Hospital is seeking a Diabetes Educator - a fast-paced individual who will provide assessment, goal-setting, education, and case management for patients and caregivers. The role focuses on promoting diabetes self-management in the inpatient and ambulatory settings. Texas Children’s offers a clinical progression ladder based on experience.
Requirements
- Provide assessment, goal setting, education, and behavioral intervention
- Evaluate patient needs and suggest management strategies
- Document all patient care activities according to guidelines
- Develop and provide education materials for patients, caregivers, and staff
- Maintain practice through continuing education
- Implement proper diabetes patient management protocols
- Perform nutritional care (RD/RDN credential)
