Job Description
The Registered Nurse role involves providing care to clients transitioning from hospitals back to the community, utilizing evidence-based Care Model of Care. The nurse will perform nursing case management, health teaching, counseling, and coordination of support services, including medication administration and documentation. Limited on-call coverage is expected.
Requirements
- NYS RN license
- Knowledge of chemical dependency as a primary diagnosis
- Excellent written and verbal communication
