The Community Coordinator is responsible for removing barriers to care for clients, providing housing, wellness, and stabilization services, and promoting consumer health. They will also support team providers, psychiatrists, and nurse practitioners with telehealth video calls and coordinate service needs, HRA applications, and medical appointments.
Requirements
- Facilitates telehealth video calls for team providers on a monthly and/or quarterly basis
- Assist with tele-visits to ensure clients are trained to do telehealth
- Create a wellness action plan tailored to each consumer’s needs
- Provide clients with ongoing follow-up regarding graduation and housing goals
- Support the team with telehealth and/or field visits, 2-person visits for high risk clients, and escorting clients to AOT appointments
- Seek out existing community medical and mental health resources and provide information and referrals for to clients as needed
- Assist clients with medication review and understanding directives from team provider in order to address mental health conditions and decrease utilization of hospitals
- Coordinate transportation resources for medical appointments, escort to appointments and provide appointment reminders as needed
