Hospice Registered Nurse Case Manager, RN

Suncrest Hospice is seeking a Registered Nurse (RN) to provide care and services for a group of patients, visiting patients in private homes, Skilled Nursing Facilities, and assisted living facilities. The RN will work in a team environment to collaborate with nurses, social workers, chaplains, volunteers, and CNA leadership.

Requirements

• Current Registered Nurse (RN) licensure (in good standing) in the state(s) of practice
• Works well within a team environment to collaborate daily with nurses, social workers, chaplains, volunteers, and CNA leadership.
• Ability to work independently and manage time effectively to manage visits, documentation, meetings, and respond to urgent needs
• Ability to learn an electronic medical records system
• Must be willing to drive with reliable transportation, valid driver's license, and auto insurance
