Social Worker II, III, Pathways to Permanency - Complex Care. Provides case management services for youth and families, creates and implements needs and services plans, and conducts weekly or bi-weekly home visits. Requires a Master's degree or LCSW, LMFT, or LPCC licensure, and at least one year of experience working with youth who have serious behavioral challenges.
