Occupational Therapist - Home Health/Hospice (0.8 FTE)

This Occupational Therapist position involves evaluating, planning, and administering Occupational Therapy services to individuals in a home setting. The role focuses on promoting independence, quality of life, and assisting patients with various aspects of their daily lives, including hand and motor skills training, functional tasks, and adaptive equipment needs. The therapist will collaborate with the interdisciplinary team, adhere to regulatory standards, and prioritize patient well-being.

Requirements

• Evaluate, plan, and administer Occupational Therapy modalities.
• Develop individualized care plans for patients with injuries, illnesses, disabilities, or cognitive dysfunction.
• Assist patients with life skills, physical or mental functioning, and adaptive measures.
• Perform detailed and accurate assessments utilizing the OASIS assessment tool.
• Participate in team meetings and case conferences.
• Maintain competency by attending continuing education.
