Southern New Hampshire Health is seeking a RN Complex Care Manager to support children and adolescents with complex medical and social needs. The role involves coordinating comprehensive, family-centered care across the continuum, partnering with pediatric patients, families, primary care providers, and community resources to address health disparities, improve outcomes, and promote self-management.

Requirements

• Current NH RN license (BSN preferred)
• Minimum 5 years of nursing experience in acute, community health, or ambulatory settings; pediatric experience strongly preferred
• Knowledge of care coordination, health coaching, and chronic disease management in children
• Strong communication, organizational, and family engagement skills
