Job Description
Soleo Health is seeking a Clinical Transition Coordinator to support patients as they transition from hospital/healthcare facilities to alternative care sites. This role involves coordinating care plans, securing referrals, assisting in patient discharge, and educating patients and healthcare providers. The ideal candidate will have experience in case management, sales support, or in a similar capacity.
Requirements
- Bachelor's degree or medical background preferred
- RN License required
- Minimum five (5) years of clinical experience
