Case Manager / PRN ( RN / RT / SW / LPN )

We are seeking a Case Manager to join our team on a part-time, PRN basis. The Case Manager will be responsible for utilization reviews, resource management, discharge planning, treatment plan management, and financial management. Must have current licensure in a clinical discipline and previous experience in an inpatient hospital setting.

Requirements

• Current licensure in a clinical discipline (RN, RT, SW, LPN)
• Previous RN/LPN/RT/SW/CM experience in an inpatient hospital setting
• Adequate experience in an acute medical case management setting
• Confidence to manage and direct a plan of care for chronically critically ill populations
