Job Description
Samaritan Daytop Village is seeking a Bilingual Case Manager to assist clients needing social service assistance. The role involves client interviews, needs assessment, creating independent living plans, locating resources, and facilitating relocation. The position supports a nationally recognized Health and Human Services Agency with over 60 programs across New York City.
Requirements
- Establishes professional relationships with clients.
- Reviews client eligibility and pre-screens for alternatives.
- Creates and maintains client files.
- Conducts initial intake and psycho-social assessment.
- Creates and maintains client files.
- Works with clients to break through barriers and advocate for self-sufficiency.
- Monitors client progress via documents and contacts.
- Follows up with referral organizations.
- Acts as a client liaison/advocate.
