Job Description
The RN - Nurse Coordinator (PT) position serves as a member of the Interdisciplinary Team (IDT) under the CMS regulations, responsible for attending IDT meetings, managing provider orders, and providing phone triage to participants.
Requirements
- Graduate of an accredited Registered Nurse Program and licensed as a professional in the State of Colorado
- Minimum of one year of health care experience in hospital, long-term or home health care setting
- CPR and First Aid certification
- Excellent written and verbal communication skills
- Proficient in software programs, such as Microsoft office and electronic medical records
- Knowledge of medical equipment and instruments
